ACCIDENT REPORTING - SUPERVISOR'S GUIDE

Supervisor's Guide - Accident investigation is an important part of a supervisor's responsibility and accident prevention. An investigation answers the questions of who, what, when, where, how, and why. A good investigation obtains as much information as possible about the accident; it may also include photographs, sketches, witness statements, and quite possibly include investigations conducted by engineering and maintenance personnel. Supervisors already know how to conduct investigations. This program will concentrate on the importance of conducting a thorough investigation and why the information is necessary. When you understand the purpose and reasons for the investigation and how the reports are used, it will make you a better supervisor and actually reduce the time you spend writing reports.

Topics included in this safety video are: the report form, the injured employee must never fill out the report, identify the employee correctly, employee's social security number, length of service, time of day, date the accident was reported and the actual date of injury, what date did the employee visit the doctor, was the treatment authorized, number of injuries or illnesses that the employee was involved in during the previous fiscal year, list all the witnesses to the accident, if there are no witnesses, description of the accident, identify the part of the body injured, the agency of accident space, if a piece of equipment contributed to the accident, unsafe act section, your opinion as to why or how the unsafe act was committed, corrective action taken or scheduled, approval signatures section

Failure to closely follow specific rules, regulations, and confined space safety standards can just about guarantee serious injury or even death. The National Institute for Occupational Safety and Health or NOISH found in more than 50 percent of confined space accidents, the rescuer becomes the victim. NIOSH also found that very few organizations provided any type of written procedures or training specific to confined space entry.

CRANE SAFETY

While there are many different types of cranes, they all have the ability to make many jobs much easier by lifting enormous weight. But they also share the potential for disaster when they are not operated safely. Crane-related accidents can often be deadly, due to the heavy loads that are lifted. A small miscalculation, or a brief moment of inattention, and tragedy could strike. Once a load falls, not much can be done to stop it, and there is little time for people to move safely out of the way. A coworker could be injured or killed, and expensive equipment or materials could be damaged or destroyed.

"Crane Safety" point out to employees that over 90% of crane-related accidents are caused by human error... and that they are the key to preventing these incidents. Topics covered in these products include:

  • Physical and mental preparation.
  • Equipment inspection.
  • Hazard assessment.
  • Boom, jib and overhead cranes.
  • General and operational safety devices.
  • Crane operations.
  • Hand signals.
  • and more.

The program comes with a comprehensive leader's guide, reproducible scheduling & attendance form, employee quiz, training log and training certificate.

    RIGGING SAFETY

We have all heard the phrase... "A chain is only as strong as its weakest link". When it comes to crane operations, rigging can often be that "weak link". Once a load falls not much can be done to stop it, and there is little time for people to move safely out of the way. A coworker can be injured or killed, and expensive equipment and cargo could be damaged or destroyed... even the crane itself.

"Rigging Safety" point out to employees that over 90% of rigging-related accidents are caused by human error... and that they are the key to preventing these incidents. Topics covered in these products include:

  • Physical and mental preparation.
