SUPERVISOR'S GUIDE TO ACCIDENT INVESTIGATION
Accident investigation is an important part of a supervisor's responsibility and accident prevention. An investigation answers the questions of who, what, when, where, how, and why. This program will concentrate on the importance of conducting a thorough investigation and why the information is necessary.

Topics included in this safety video are: the supervisor's report of injury - make sure your reports are legible, either printed or typed, the employee name, location, sex, age, identify the employee correctly, employee's social security number, length of service, time of day, date the accident was reported and the actual date of injury, number of injuries or illnesses that the employee was involved in during the previous fiscal year, list all the witnesses to the accident, description of the accident, what the employee was doing prior to the incident and what the employee was doing during the incident, identify the part of the body injured, the object, substance, or body motion which directly produced the injury, identify the specific piece of equipment, including the serial number that contributed to the accident, the Unsafe Act section, your opinion as to why or how the unsafe act was committed and list actions to eliminate the contributing factors, unsafe acts, and conditions that caused the accident.
